Indigenous Health Outreach Program
The Indigenous Health Outreach Program at Hawkesbury-Hills Division of General Practice is funded by the Australian Government Department of Health and Ageing to provide practical assistance to Aboriginal and Torres Strait Islander people in accessing healthcare services. Once referred to the Indigenous Health Team, the Outreach Worker will assist the referred client with the following as required:

Finding a local general practitioner to provide regular care where one is not identified by the client;

Booking and confirming healthcare appointments;

Accessing transport for healthcare appointments;

Providing general support at a healthcare appointment, including completing healthcare forms.

Indigenous Health Incentive – Practice Incentive Program
On 1 May 2010 the Department of Health and Ageing introduced the Indigenous Health incentive for accredited general practices through the Practice Incentive Program (PIP). The Indigenous Health Incentive aims to support general practices to provide better health care for Aboriginal and Torres Strait Islander People. In particular, the incentive is focussed on improving the management of chronic disease. Although the incentive is only for practices accredited with the RACGP, it is important that non-accredited practices are aware of the incentive as you may receive questions from patients about this initiative.

Indigenous Health Incentive and Medical Item Number Income Calculator
The Indigenous PIP calculator was developed by Moreton Bay General Practice Network to assist Practice Owners, Managers, GPs and staff to calculate the potential Medicare income derived from Indigenous Health Incentive payments. The relevant MBS items needed for Tier One and Tier Two payments are also included.

Cultural Awareness Training Provided
As a requirement of the Indigenous Health Incentive, registered general practices must nominate at least two practice members (one of whom must be a GP) to complete cultural awareness training within 12 months from registering for the incentive. Please note, for practices that registered for the incentive before the release of the RACGP Online Culteral Awareness in April 2011, the RACGP granted an extension of 12 months from the release date of the training to complete the cultural awareness training requirements.
